This form can only be used to make an online payment for one of the following OPUS forms:
- Chapter Personnel Report
- Chapter Personnel Update
- Initiate Registration
- Honorary Initiation
- Chapter Installation Form
- Inactive Alumni Reinstatement
IMPORTANT: Do not complete this form unless your submission has been approved in OPUS by your Sponsor & Director of Bands, and you have a PDF copy of the form you are submitting payment for.
You can use the print to PDF option on your computer, or print and then scan a copy of then form to create a PDF. If you do not have access to a scanner, you can use a mobile app such as CamScanner to create the PDF from a cell phone. Your Summary Page must be attached to this form before clicking the Pay Now button. (Disregard the references to Donation on this form.)
Double-check the amount due on your form in OPUS before proceeding. Complete payment must be submitted when using this form. It should not be used for individuals to submit their own dues.

All due dates and late fees remain in effect regardless of payment method.
